Tŷ Brian is Summit Care Services first residential home and offers a new and specialised service that will provide a supportive home environment to individuals with complex mental health needs who have found themselves or are at risk of finding themselves in conflict with the criminal justice system. They will have a mental disorder and possibly a mild learning disability, and all could present challenges to themselves and the public if not professionally supervised.

We seek to support our residents to maximise their own potential by supporting them to develop their skills towards the goal of independent living. Our approach is based on respect, dignity and fulfilment for the individual whilst maintaining constant and discreet supervision. Our practice is based upon the Active Support model and adopts the variant of Person Centred Planning known as Essential Life Planning.

The team at Tŷ Brian is focused on ensuring that our residents engage appropriately and become part of their local community and can access a full range of educational, social and recreational facilities to meet their needs and aspirations. Our service is located in a landmark location close to the centre of Wrexham with excellent access to public transport, local shops and social, recreational and educational amenities.

Person Centred Care

We are advocates for the Active Support model of support.

Active Support is a way of supporting people to take part in meaningful, everyday activities. It might include anything from making a meal, gardening or cleaning the bathroom to playing golf, going to the bank or visiting the theatre. The difference is that we provide just enough support for you to take part in everyday tasks. Although active support is not about teaching, people do learn new skills by taking part. Our residents don’t have active support “sessions”; people we support don’t “do” active support. It is something that will happen naturally and should, where possible, be led by our residents.

Each individual resident here will have his own tailored daily activity plan, his own set of care plans and individualised risk assessments.

Family Matters

The individuals whom we care for will normally be part of a wider network of family, friends and other social groups. If they are important for our residents then they are important to us.

We encourage our residents to identify family and friends who care about them and who wish to maintain a relationship. We will work with them to ensure we provide the appropriate service for their relative and also to support them in their own right as an aid to recovery.

Service User Involvement

Good rehabilitation services rely upon acting upon the views of service users.

Tŷ Brian will be home for the 6 individuals who will be resident with us. It is imperative that they feel able to have a say in how the home is run and we will encourage and support them to say how they feel about all aspects of using the service. We will listen to their views about how we support them and also about aspects of service delivery such as activities and day-to-day opportunities. All residents will take part in:

  • The weekly house meeting.
  • Monthly key worker meeting.
  • The manager’s daily “Talk Time” programme.

Personal Activities Plan

We will talk to our residents and those with whom they have links and we will identify their needs, interests and aspirations. Each resident will then have their own tailored daily activity plan, his own set of care plans and individualised risk assessments.

Our residents are supported to participate in social activities that may be of interest to them through the well-established network of community-based groups.

We will support them to develope independent living skills such as financial planning, budgeting for personal shopping, meal preparation and use of public transport. These skills will enable to have confidence when the time is right for them to move on to more independent or supported-living accommodation.

Service Staffing

The service is staffed 24 hours a day, seven days a week. The team is made up of an experienced Registered Manager and a full team of support workers.

The service has full access to primary care and to a range of specialist professionals including psychiatry, psychology, occupational therapy and speech therapy as required.
